What the form does now
The default form displays a request preview. Your entries remain in the current page’s memory and are cleared when you navigate away or reload. They are not submitted to Zealous, stored in the browser, emailed, used to create a member record or used to reserve an appointment. Do not provide medical information.
Your choices
This service is for adults 20+. An authorized contact may inquire with the adult’s permission; paying or enquiring does not automatically grant access to private records. Service contact permission is separate from marketing; the form does not subscribe you to marketing.
Assets and technical access
Google Fonts supplies typography with system fallbacks. Imagery is hosted with the site and is AI-generated, not actual clients, staff or premises. No advertising pixels or analytics have been added. The hosting service may process ordinary technical access information under its own terms.
Before public operation
The operator must confirm business identity and contacts, processing and retention policies, processors, rights and actual service terms. Enabling a secure inquiry endpoint requires an accurate updated privacy policy, server validation, protected staff access and genuine follow-up. Private clinical screening uses a separate appropriate process.
